Carcinomatosis is one of the most formidable options for the progression of a number of oncological diseases. Carcinomatosis is a form of metastasis in which tumor cells spread along the inner lining of the anatomical cavity (abdominal or pleural) and form small nodules on it, the so-called disseminated. Its occurrence in most cases implies stage IV of the disease, and is associated with an unfavorable prognosis for the patient. The treatment of patients with peritoneal carcinomatosis is one of the most difficult tasks in oncology. It most often develops in malignant neoplasms of the gastrointestinal tract, respiratory system and female reproductive organs.

Effective treatment approaches usually involve combination therapy, for example, a combination of surgery and systemic chemotherapy. One of the most effective methods is local chemotherapy. The idea of local chemotherapy is to ensure the delivery of the drug directly to the tumor cells located on the inner lining of the anatomical cavity. Due to the extremely low permeability of this area for drugs, for example, when they are administered intravenously or taken in the form of tablets, the delivery of drugs in the form of an aerosol under pressure of carbon dioxide gives the accumulation of the drug in the tumor in significantly higher concentrations.

Methods of treatment of patients with carcinomatosis

For a long time, carcinomatosis of the peritoneum and pleura was considered a terminal stage of the disease, not subject to special treatment. The development of techniques for direct exposure to the peritoneum and pleura has improved the prognosis for this condition. The Oncologists of the Hertsen Institute has unique capabilities for the treatment of patients with carcinomatosis, having in its arsenal the whole complex of the latest methods of therapy.

This is a technique in which, during surgery, a highly concentrated solution of chemotherapeutic drugs heated to 42.5 oC is injected into the abdominal or pleural cavity, which ensures their effect directly on cancer cells in the abdominal (pleural) cavity with minimal impact on other organs. The session is held for an hour, which allows you to destroy the tumor cells left after the so-called cytoreductive surgery.

This is the latest innovative method for the treatment of peritoneal carcinomatosis, which ensures the delivery of the drug directly to the tumor cells located on the inner lining of the anatomical cavity. The method has recently appeared in Russia. The procedure is performed by laparoscopy (thoracoscopy). In the abdominal or pleural cavity , an aerosol of chemotherapy drugs is sprayed under pressure of carbon dioxide and left for 30 minutes . With this method of administration of chemotherapy drugs, their effect on the tumor increases many times in the complete absence of systemic toxicity. The know-how of the P.A. Hertsen MNIOI in this method of treatment was the development of its own special nozzle, which forms a flow of fine aerosol with a droplet size from 1 to 40 microns. The product is made of specialized materials and is not inferior in quality to its imported counterpart, and surpasses it in a number of technical characteristics.

Aerosol chemotherapy

The method of applying aerosol chemotherapy under pressure (PIPAC) was first tested in Germany, and the P.A. Hertsen Moscow Research Institute of Oncology – a branch of the NMIC of Radiology of the Ministry of Health of Russia in 2013 became the second center in the world to start studying and implementing this technique. The large-scale scientific and practical work carried out over the past 6 years has allowed the Institute to take a leading position in this field and be the first in the world to prove the feasibility of using aerosol chemotherapy as one of the main methods of treatment in patients suffering from peritoneal carcinomatosis.
