SURGEONS OF THE NMRRC PERFORMED A COMPLICATED OPERATION ON A PATIENT WITH OBESITY

A 60-year-old patient with extreme aggression and obesity was admitted to the Department of Reconstructive-Plastic Gynecology and Oncology of the N. Lopatkin Scientific Research Institute of Urology and Interventional Radiology (SRIUIR) – the branch of the FSBI “National Medical Research Radiological Centre” of the Ministry of Health of the Russian Federation. In addition to excessive weight of 140 kg with a height of 155 cm, the woman suffered from a ventral hernia developed after gallbladder removal and ectopic pregnancy. In addition, the situation was complicated by the presence of varicose veins of the lower extremities with trophic ulcers and diabetes mellitus.

She was diagnosed in Stavropol Region, where the patient is from, and needed emergency surgical care. However, the possible risks of serious postoperative complications against the background of concomitant aggravating factors made it impossible to carry out treatment at her place of residence. The woman received a referral for consultation at the NMRRC.

A surgical treatment tactic was developed at the oncologic consultation with the participation of Andrei Kaprin, Director General of the National Medical Research Radiological Centre of the Ministry of Health of the Russian Federation and the Heads of the Surgical, Drug and Radiation Departments.

The surgical team was headed by Professor Anatoly Gritsai, Head of the Department of Reconstructive Plastic Gynecology and Oncology, together with Dmitry Tsaava, Head of the Minimally Invasive Pelvic Surgery Group of the N. Lopatkin SRIUIR. The specialists managed to perform the most complicated multistage surgical intervention, including removal of the skin and fat apron (weighing 25 kg), extirpation of the uterus with appendages, plasty of the anterior abdominal wall with a mesh implant and abdominoplasty. The surgery, which lasted more than 5 hours, was completed successfully, and the woman was discharged home on the 10th day.

“Patients with such an aggravated anamnesis, like our heroine, are always the prerogative of the federal center. We have all the necessary conditions to provide high-tech care, including those requiring the participation of a multidisciplinary team of specialists,” Anatoly Gritsai noted. “Given the highly aggressive course of the disease, the patient will undergo further courses of drug treatment at her place of residence, but she definitely does not need surgical treatment in the near future,” concluded the patient’s attending physician, oncologist Zalina Sidakova of the Department of Gynecologic Oncology.
