THE SURGICAL TEAM AT NMRRC RADIOLOGY SAVED A PATIENT FROM TWO LIFE-THREATENING DISEASES
Valery, an 85-year-old man, began to notice a prolonged hoarseness of voice that had been bothering him for several weeks and had not been treated with medications. He went to the lorist at his place of residence and the doctor diagnosed a malignant neoplasm of the larynx.
As a rule, in such a situation surgical and radiation treatment is indicated. But in Valery’s case, it was associated with an extremely high risk for life, as he had a number of complex concomitant cardiovascular diseases, including carotid artery stenosis of more than 85%. This case required routing the patient to specialists at a federal center.
The doctors referred Valery to the A. Tsyb Medical Radiological Research Centre (MRRC) – the branch of the FSBI “National Medical Research Radiological Centre” (NMRRC) of the Ministry of Health of the Russian Federation , which has not only a strong team of oncologists, but also vascular surgeons who perform surgical interventions of any complexity.
“We thoroughly further examined our patient and realized that there is no reason to deny him surgical and radiation treatment. Medical care of such comorbid (with multiple pathologies) patients requires a special approach with the participation of doctors of different specialties. After a multidisciplinary consilium, it was decided to perform carotid artery surgery as the first stage, followed by laryngectomy (removal of the larynx with adjacent tissues) and modified cervical dissection,” says Ernest Akki, oncologist and surgeon of the Head and Neck Diseases Department of the A. Tsyb Medical Radiological Research Centre.
The operation went according to plan and lasted 5 hours. During this time Vladimir Iofik, vascular surgeon of the A. Tsyb Medical Radiological Research Centre, performed eversion carotid endarterectomy (removal of atherosclerotic plaques from the carotid artery). The second stage Ernest Dilyaverovich removed the larynx, adjacent tissues and lymph nodes.
The operation went as planned and lasted 5 hours. A great role in achieving favorable results in such surgical interventions is given to the anesthesiology team, which during the operation applies special methods of cerebral blood flow control and monitors the patient’s condition.
After such an intervention, 85-year-old Valery feels satisfactory. In a few weeks he will come to the A. Tsyb MRRC for radiation therapy.
