At birth, Said was found to have a defect in the valve of the posterior urethra. A congenital abnormality of the development of the urinary tract caused the beginning of the death of the renal parenchyma. In 1999, they tried to help him – they brought the ureters out from both sides to expand them. Additionally, the folds of the posterior urethra were dissected. Adequate urination was restored for a while. But even then, doctors noted an increase in the figures of azotemia, that is, a steady decline in kidney function. In 2001, the ureterocutaneostomy was closed on both sides to exclude possible infection from the outside. However, already in December 2008, due to the increasing retention of urine in the bladder, botulinum toxin type A was administered to relax it and reduce pressure. It helped for a while.
The whole life– struggle is about him. He was treated for 24 years. From chronic kidney disease in the nephrology department, in the Department of urology for periodic exacerbations of chronic pyelonephritis. In November 2019, the patient turned to the doctors again. During the examination, the indicators of azotemia rose to 350 mmol/l, proteinuria to 1 gram per day and Said was hospitalized again.
At the time of admission to the Research Institute of Urology and Inteventional Radiology named after N.A. Lopatkin, a branch of the FSBI “NMIC Radiology” of the Ministry of Health of Russia, he was diagnosed with chronic kidney disease of stage 5. Dialysis is needed. At the age of 24, complete dependence on the disease. The kidneys practically failed.
It can’t go on like this anymore, Said’s mom decided. Rather than going to procedures all your life, being tied to them, it’s better… She had known for a long time what was best. A new kidney. A healthy organ that will radically change the life of the son and the whole family. Said’s mom made a decision: save your son, break this vicious circle of addiction, give him a second life! Give away your kidney.
Related transplantation. It used to be only in books. Today, this is a reality in transplant departments in five or six centers in Russia, “survivors” during the restrictions of the coronavirus.
– In total, about two hundred such operations are performed in the country per year, – says Sergey Arzumanov, head of the transplantation and renal replacement therapy group at the Research Institute of Urology and Interventional Radiology, a branch of the Federal State Budgetary Institution “NMIC Radiology” of the Ministry of Health of Russia.
– Before the pandemic, we performed up to 60 kidney transplant operations per year. Today, due to the tense epidemiological situation, this figure has halved. Kidney transplant operations have become isolated cases, although the need for them has increased significantly. If earlier a person who needed to replace a kidney was on the waiting list for two or three years, now the period has increased to five years. There are practically no donors.
In five years, Said would have been thirty. No family, no children, no normal work. And Mom decided to become a donor.
According to experts, kidney transplantation performed from a living donor has the best and safest result for the recipient, provides minimal likelihood of complications in the postoperative period and good function of the transplanted organ immediately after the intervention.
– We performed laparoscopic sampling of the donor kidney together with the ureter. This is a standard practice at our institute, applied for 10 years,” Dr. Arzumanov continued, “it minimizes the surgical trauma of the donor, and ensures his rapid rehabilitation. Usually the donor is discharged from the hospital 5-7 days after the operation.
Next, we performed a fairly standard kidney transplant operation, which is routine for us,” the surgeon concludes, “we will only have to wait for the young body to quickly adapt to the new reality and recover.
The new kidney will help the young man to return to a normal social life, and, hopefully, will save him from constant torment and endless trips to the doctor.
The donor kidney transplant operation was carried out normally, the mother and son have already been discharged and went home.
