Female Reproductive System Cancers

Cancer of the female reproductive system usually develops against the background of hormonal imbalance. This may involve an altered ratio of estrogens and gestagens and can be either relative or absolute. Most often, cancer is diagnosed due to an excess of estrogen hormones, which provokes excessive cell proliferation and growth. Under these conditions, healthy cells may transform into malignant ones.
The presence of human papillomavirus (HPV) in a woman’s body also plays a significant role: certain HPV strains can trigger cervical cancer. Only some HPV strains have this effect — they can integrate into the DNA of healthy cells and initiate their transformation into malignant cells.
Risk Factors That Increase the Likelihood of Tumors in the Female Reproductive System

  • Excess body weight — adipose tissue stimulates increased estrogen production.
  • Menstrual cycles without the formation of mature ova.
  • Early onset or late cessation of the menstrual cycle (at 12 and 55 years, respectively).

Symptoms and Signs of Cancer of the Female Reproductive System

Malignant tumors of the female reproductive system are an umbrella term for various oncological diseases affecting the organs of the reproductive tract. Specific symptoms depend on the site where healthy cells begin transforming into malignant ones:

  • Cervical cancer. Characterized by vaginal bleeding after sex or after inserting vaginal suppositories, mild pain and discomfort in the lower abdomen, nighttime lower back pain, and frequent urges to urinate.
  • Endometrial cancer. Main symptoms include bleeding between menstrual periods, pain in the lower back and lower abdomen. Vaginal discharge may persist even after menopause, often becoming colorless.
  • Ovarian cancer. Often asymptomatic; the condition is usually detected incidentally. A woman may experience only mild, poorly localized lower abdominal pain.
  • Cancer of the external genital organs. Symptoms are typically pronounced: itching and burning in the perineal area; as the disease progresses, bleeding ulcers develop. These signs are easily detected during a superficial examination of the external genitalia, which allows treatment to be started at an early stage.

Cancer in any organ of the female reproductive system is easiest to treat at an early stage. For this reason, regular preventive gynecological checkups are essential. Any of the following symptoms should prompt immediate consultation with a qualified healthcare provider:

  • Menstrual cycle disturbances — periods that are unusually long or short.
  • Recurrent severe lower abdominal pain occurring outside the menstrual period.
  • Vaginal discharge of purulent, mucous, or bloody nature, accompanied by an unpleasant odor.
  • Itching and burning sensation in the perineal area.
  • Development of papillomas or condylomas on the skin of the body.
  • Urinary disturbances — excessively frequent urges, a strong feeling of pressure on the bladder.

Symptoms of cancer in the female reproductive system may be mild and intermittently disappear. Therefore, any issues with the reproductive system — from heavy menstrual bleeding to abdominal cramps — should be a cause for concern.

Diagnosis of Cancer of the Female Reproductive System

A gynecological examination alone is insufficient to diagnose tumors of the female reproductive system. Symptoms that raise concerns also cannot be used as the sole basis for a diagnosis. A set of specific tests is required:

  • Pap test (Papanicolaou smear). Highly informative for detecting tumors at an early stage. The optimal time for the test is 5 days before or 5 days after menstruation.
  • Biopsy and colposcopy. Performed if cervical erosion or any changes in cervical epithelial cells are detected. These methods help identify cancer at an early stage, increasing the chances of preserving the functionality of the female reproductive system after treatment.
  • Transvaginal ultrasound. Allows the physician to determine both the exact location and the size of the tumor. This method can detect cancer even in the absence of symptoms.

To confirm the diagnosis, additional examinations are prescribed: puncture of the posterior vaginal fornix, endometrial curettage, computed tomography, and abdominal ultrasound. Only after the diagnosis is confirmed and the tumor is fully assessed will the doctor develop a treatment strategy.

Treatment Methods for Cancer of the Female Reproductive System

The optimal approach to treating cancer of the female reproductive system is tumor removal followed by drug therapy. The latter is necessary to destroy any remaining atypical cells and prevent recurrence, as completely removing the neoplasm is often difficult. The treatment plan is tailored to the tumor size, location, overall condition of the reproductive system, and disease stage. The most effective methods include:

  • Chemotherapy. Destroys tumor cells and metastases, even if they were not previously detected.
  • Radiation therapy. The entire organ is targeted both externally and internally; a combined approach is often used.
  • Hormone therapy. Since cancer growth may continue if hormonal imbalance persists, sometimes an artificial menopause is induced to cause tumor atrophy and halt its growth.
  • Immunotherapy. Targets pathogenic cells that may remain hidden and mimic healthy cells for long periods.

Palliative treatment is prescribed when cancer is diagnosed at advanced stages and even surgical removal does not lead to improvement. In such cases, the goal is to alleviate the patient’s condition and reduce symptom severity. However, this type of therapy does not stop tumor growth.

Prognosis for malignant tumors of the female reproductive system varies. If cancer is detected at an early stage, the probability of complete recovery is about 80 %. However, stage II, with involvement of lymph nodes, has a more uncertain prognosis, while stage IV, with spread of atypical cells to internal organs and systems, carries an unfavorable outlook.
Even successful treatment of cancer of the female reproductive system can affect future fertility:

  • Chemotherapy drugs can destroy egg cells.
  • Ovaries may age prematurely or partially atrophy.
  • Menopause may occur much earlier than the natural biological timeframe.
  • Some therapies can cause scarring of uterine tissue.
  • Surgical removal of the tumor may damage reproductive structures.

The effectiveness of treatment for cancer of the female reproductive system directly depends on timely diagnosis. The disease is considered treatable, and cancer is no longer a death sentence: modern methods allow not only to eliminate the malignant tumor completely but, in most cases, to preserve the functions of the female reproductive system.
