
TOTAL OR SUB-TOTAL GLOSSECTOMY WITH MICROSURGICAL RECONSTRUCTION + LYMPHDENECTOMY ON BOTH SIDES OF THE NECK
More than 100 surgeries for cancer removal and reconstruction of tongue functions have been performed in the microsurgery department at P. Hertsen Moscow Oncology Research Institute (MORI) – branch of the National Medical Research Radiological Centre (NMRRC) of the Ministry of Health of the Russian Federation. This is the greatest experience in this field. We have patented our own method of restoring the functions of this organ using so-called reinnervated chimeric autografts.
Within the last years cancer of the oral mucosa has kept the leading position in the overall structure of oncology diseases. Mortality of the tongue cancer in the 1-st year after establishing diagnosis makes up 46,5 %. The leading in treatment of patients with such disease is a combination method, the key stage of which is radical surgery remove of the tumor. Execution of surgery of this volume is surely bound with formation of extensive defects of the oral cavity and tongue and loss of necessary body functions. In this situation issues of single-step reconstruction become significant.
Method of single-step elimination of the tongue and oral cavity floor
In P. Hertsen MORI – branch of the FSBI NMRRC of the Ministry of Health of Russia has been developed and brought into practice a method of single-step elimination of the tongue and oral cavity floor defect after subtotal/total glossectomy in patients with III-IV stages of tumor process.
The method consists of reconstruction of the tongue and removal of the defect of mouth cavity floor with a revascularized, reinnervated thoracic-dorsal “chimeric” flap with the inclusion of the serratus anterior muscle and the broadest muscle of back simultaneously with the resection stage
