Reference Center

In December 2019, a reference center for molecular genetic, immunohistochemical, pathomorphological and radiation research methods was established on the basis of the FSBI “NMIC of Radiology” of the Ministry of Health of the Russian Federation by Order No. 129 of 12/24/2019, in order to ensure the implementation of the action plan of the federal project “Fight against Oncological Diseases”.

The main goal: Histological and molecular genetic verification of the detected neoplasm in patients, determination of the optimal possibilities of drug therapy for each patient, taking into account his specific disorders.

Tasks and functions:

  1. Consultations on the interpretation and description of the results of pathomorphological, immunohistochemical, molecular genetic and radiation studies of malignant neoplasms;
  2. Conducting pathomorphological, immunohistochemical, molecular genetic and radiation studies in order to verify the diagnosis and personalized choice of strategies and tactics for the treatment of oncological diseases;
  3. Prospective testing of tumor tissue samples to identify known mutations that are targeted for targeted drugs.
  4. Ensuring the rapid inclusion in the testing scheme of new genetic disorders necessary for the optimal selection of therapy for a particular patient.

The reference Center of the Federal State Budgetary Institution “NMIC of Radiology” of the Ministry of Health of the Russian Federation conducts an extensive list of molecular genetic studies in cooperation with medical institutions for their patients.

How to refer a patient.

1.A number of molecular genetic studies can be performed at the Federal State Budgetary Institution “NMIC of Radiology” of the Ministry of Health of the Russian Federation, to any resident of the Russian Federation, at the expense of the territorial funds of the CHI, if there is a referral in the form 057/y-04.

For registration of MHI services, you need the correct direction in the form 057 /u -04.

Reference Center

In addition to the form 057/y, the following data for the patient must be provided for registration of MHI services:

  • a copy of the patient’s passport (cover page and registration page);
  • a copy of the patient’s CHI policy on both sides;
  • copy of SNILS;
  • informed consent signed by the patient.

2.Studies that are not paid for under the compulsory medical insurance system are performed under a corporate agreement between the Federal State Budgetary Institution “NMIC of Radiology” of the Ministry of Health of the Russian Federation and a medical institution. The timing of molecular genetic studies is from 5 to 14 working days from the date of receipt of the biomaterial by the laboratory, depending on the complexity of the study and the quality of the biomaterial.

  1. Histological examination (revision of glasses and histoscans, as well as a description of the results of pathoanatomic examination of biopsy material).
  2. Immunohistochemical examination of histological material by various markers.
  3. FISH examination of HER2 biopsy material for diagnoses of “malignant neoplasm of the breast” and “malignant neoplasm of the stomach”.
  4. FISH studies of ALK and ROS1 biopsy material in the diagnosis of lung cancer.

It is possible to consult histological scans.
Consultation of histological scans is possible on the basis of the following services (pre-registration is required):

To conduct a consultation, it is necessary to create a request in the telemedicine system and upload the patient’s histoscans to one of these services. The reference center of the Federal State Budgetary Institution “NMIC of Radiology” of the Ministry of Health of the Russian Federation is reviewing the results of radiation research methods as part of a request for telemedicine consultations in the “doctor-doctor” format.

Rules for providing telemedicine consultations on the profile of “Radiation diagnostics”

  1. Research should be sent to the reference center in DICOM format
  2. The forwarded patient data should be anonymized when downloading from PACS
  3. The data provided for consultation must necessarily have accompanying medical documentation (referral, anamnesis, clinical laboratory and instrumental data)
  4. The purpose of the consultation should be indicated in the direction:
    • discrepancy of anamnesis, clinical / instrumental / laboratory data with the data of radiation diagnostic methods;
    • discrepancy between conclusions when performing two or more methods of radiation diagnostics (CT/MRI/PET-CT);
    • difficulties of interpretation of the study at the place of its conduct (including differential diagnosis of detected changes);
    • clarification of the prevalence of the identified tumor process (staging and restading)
    • evaluation of the effectiveness of the therapy (evaluation of dynamics).
  1. The directed research should be of proper quality:
    • performed with contrast enhancement using automated injectors, in compliance with the rules of contrast for a specific organ/area (dynamic contrast enhancement, multiphase with respect to time intervals, etc.)
    • the study area covers the entire area of “interest”, including the potential area of regional lymphogenic metastasis
    • MRI scanning protocols recommended by experts of international committees.

How to transfer biological materials for research

There are 2 possible options for transferring biomaterial for molecular genetic, immunohistochemical and pathomorphological studies to the reference center of the FSBI “NMIC Radiology” of the Ministry of Health of the Russian Federation.

  1. The patient personally transfers the biomaterial and documents to the registry of any branch of the FSBI “NMIC of Radiology” of the Ministry of Health of the Russian Federation at the following addresses:
    • Moscow, 2nd Botkinsky Ave., 3.
    • Moscow, Pogodinskaya str., 6, p. 1.
    • Moscow, 3rd Parkovaya str., 51, p. 1.
    • Obninsk, Koroleva str., 4
  2. The patient applies to the medical institution to which he is attached, at the place of residence.

In this medical institution, the patient is issued form 057 / u -04, and, after preparing the necessary package of documents (a copy of the patient’s passport (title page and registration page); a copy of the patient’s CHI policy on both sides; a copy of the SNILS; informed consent signed by the patient; clinical information), courier delivery from the medical institution to the FSBI is organized “NMIC of Radiology” of the Ministry of Health of Russia, by the reference center.
